EHTIYOJLARNI BAHOLASHDAN OILAGA YORDAM KO‘RSATISHGACHA

    Toshkent shahrida istiqomat qiluvchi ijtimoiy himoyaga muhtoj oilaga ijtimoiy xodim tomonidan kompleks ijtimoiy yordam ko‘rsatildi.Dastlabki bosqichda oilaning ijtimoiy holati va yashash sharoitlari o‘rganilib, mavjud muammolari hamda ehtiyojlari aniqlandi. Oilaning individual ehtiyojlarini hisobga olgan holda uning holati kompleks baholandi.Baholash natijalariga ko‘ra, aniqlangan muammolarni hal etish bo‘yicha keyingi ishlarning asosiy yo‘nalishlari, zarur ijtimoiy qo‘llab-quvvatlash choralari hamda amalga oshiriladigan tadbirlar ketma-ketligini belgilovchi individual yordam rejasi ishlab chiqildi.Oilani keyingi bosqichlarda ijtimoiy kuzatib borish va qo‘llab-quvvatlash nazarda tutilgan.

FROM NEEDS ASSESSMENT TO INDIVIDUALIZED FAMILY SUPPORT

A social worker from the Republican Center for Social Adaptation of Children provided comprehensive support to a socially vulnerable family in Tashkent. The work began with a detailed review of the family’s circumstances, including their living conditions, current situation, and the difficulties they were experiencing.

The specialist assessed the family’s needs as a whole while also taking into account the individual circumstances of its members. This helped identify the main areas requiring attention and determine what type of assistance would be most appropriate.

Based on the assessment, an individualized support plan was prepared for the family. The plan sets out the main areas of work, the social assistance and other measures required, and the steps to be taken to address the identified concerns.

To ensure that the family continues to receive the necessary assistance, ongoing social support has also been arranged. This approach allows specialists to work with the family’s specific needs in a consistent and structured way and to monitor the support provided over time.

От оценки потребности к помощи семье

             Социальным работником оказана комплексная социальная помощь социально уязвимой семье, проживающей в городе Ташкенте. На первоначальном этапе были изучены социальное положение и условия проживания семьи, выявлены имеющиеся проблемы и потребности. Проведена комплексная оценка ситуации семьи с учётом их индивидуальных потребностей. По результатам оценки разработан индивидуальный план помощи, определяющий основные направления дальнейшей работы, меры необходимой социальной поддержки и последовательность действий по решению выявленных проблем. Предусмотрено дальнейшее социальное сопровождение семьи.
